Forms

Drop form

Students must drop upper-level courses by the end of the third week of the quarter. All upper-level adds and drops during the first five days of each quarter are done using the telephone system or via the student's myNEU account (http://myneu.neu.edu). After the first five days, students must complete a drop form when dropping upper-level courses and submit the form to the Office of Academic and Student Affairs.

Independent Study form

Forms for independent study must be signed by the supervising faculty member as well as Assistant Dean Bettye Freeman. Completed independent study forms must be filed with the Office of Academic and Student Affairs during the first five days of each quarter. All independent study proposals must contain a written summary of the proposed projects. Independent study projects may be granted for no more than three (3) quarter hours each. No more than three quarter hours of independent study credit may be earned in any one academic quarter. Students are limited to a maximum of six (6) quarter hours of independent study credit during their upper level academic quarters.

Over/Under form

Each student is advised to register for 12-16 quarter hours each academic quarter. In order to take an upper-level course load of less than twelve (12) or more than sixteen (16) quarter hours, a student must complete an over/under form and submit it to the Office of Academic and Student Affairs. This is done in order to maintain residency status and to ensure that you are not taking on more than you can accomplish successfully. (Students must be registered for a minimum of 10 quarter hours per quarter and of those 10 quarter hours successfully complete 9 quarter hours of course work to meet the ABA residency requirement.)

Change of Division (Rotation)/Status form

Deliver completed form to the Office of Academic and Student Affairs.  Requests are considered by a Committee during the second and ninth week of the quarter.  Notice of approval/denial will be forwarded to you by mail.

Change of Address form.

The Office of Academic and Student Affairs needs your current and permanent addresses to mail all correspondence including evaluations and registration materials.

Academic Credit Calculation Worksheet

Academic Requirements: Students are responsible for keeping track of their law school academic credits and requirements. Although the office can review your credits with you at any time, we have developed an Academic Credit Calculation Worksheet that will assist with this task.

Exam Review Request

If a student would like to sign out an exam answer, you may make this request in writing.  The form is available in the Office of Academic and Student Affairs.  Deliver the completed form to the office allowing us two days to fill your request.

Upper Level Exam Reschedule Request

Students may petition to reschedule an exam if they have two exams on the same day or three exams on consecutive days. Rescheduled exams must be taken within the designated reschedule periods.

Diploma Information

The Office of Academic and Student Affairs provides the University Registrar with the list of students for May, September and January commencements. The office clears each student by ensuring that copies of all course and co-op evaluations are on record in the student's file, and that all law school requirements are fulfilled. The office also provides students with a form to complete indicating how you wish your name to appear on your diploma.  Completed forms are due to the office during the winter quarter of the student's third year.
